Several Black Clover fans ask themselves the question "Are Lucius and Astaroth the same person?" based on the way they were introduced in the story and the role they play in the final arc of the manga. However, it can be confirmed from the get-go that they are not the same person and play different roles throughout the story, even if they are fully connected.
It is also worth pointing out that Astaroth has not made a physical appearance in Black Clover, as of this writing, and has only been mentioned thus far. That, though, has not stopped the character from being extremely important to the plot, to the point that Lucius Zogratis' plans wouldn't have worked without the Devil's input and abilities.

Astaroth and Lucius' relationship in Black Clover, explained
Astaroth and Lucius are not the same person, and that has been confirmed throughout the final arc of author Yuki Tabata's manga. The former is one of the three Devils who ruled the underworld and, at some point, walked away from that reality and ended up meeting two very important players in the story's narrative, Julius Novachrono and Lucius Zogratis.
Julius and Lucius were twin brothers trapped in the same body, with the latter being the oldest sibling of the Zogratis household, who were the antagonists of the Dark Triad arc. Lucius showed a lot of promise as a devil host and struck a contract with Astaroth, thus gaining the Time Magic ability that gave Julius the chance to become the Wizard King.
This role as the Wizard King also meant that Lucius was now in a very prominent position of power and could plan his magic purity goal with care and detail.
Lucius' role in the story
Lucius' role in the story of Black Clover is monumental, as he orchestrated the plans to weaken Lucifero in the Dark Triad arc and was also responsible for Julius Novachrono's rise as the Wizard King. He is also, on paper, the final villain of the manga and the biggest ordeal that Asta and his friends had to face thus far in the story.
His plan of magical purity is also the most ambitious goal that any villain in the story has had, and he played the long game in that regard, manipulating events with his Time Magic so events could play out the way he wanted to. It goes to show that even Julius turning into a child was a part of his plan and managed to attack the Clover Kingdom when they were at their weakest.
Astaroth's role in the story
It is worth pointing out that Astaroth has never made a physical appearance in the entirety of Black Clover and has only been mentioned a couple of times. However, his importance to the story is monumental by the fact he was the Devil who struck a deal with Lucius Zogratis, thus giving him the power to execute his plan.
If it wasn't for Astaroth's influence, Lucius wouldn't have the Time Magic to manipulate events and Julius wouldn't have become the Wizard King. In that regard, there is a very good chance that the entire story would have been entirely different if it wasn't for Astaroth.
There have been some theories in the fandom that this Devil is going to be the final boss of the series, although that is mere fan speculation at the moment.
