In Black Butler season 4 episode 9, Derrick Arden's unexpected appearance at the Midnight Tea Party shocks Ciel Phantomhive and the four Prefects.
Derrick's bizarre behavior escalates as he attacks Herman Greenhill, biting off his arm. Observing a stitch mark on Derrick's forehead, Ciel realizes Derrick resembles the undead creatures from the Campania. He commands Sebastian to subdue Derrick, who then tends to Herman's wounds.
Ciel then reveals his true purpose at Weston College and Sebastian's role as his butler. He deduces that Derrick has been dead for a while, exhibiting animalistic behavior. Seeking answers, Ciel confronts the headmaster, but Herman reveals the Prefects' secret: they killed Derrick to maintain order.
A flashback shows their panic and subsequent deal with Rian Stoker of the Karnstein hospital to revive Derrick, involving the Aurora Society and revealing the headmaster as Undertaker, a Grim Reaper.
Sebastian demands an explanation for the Prefects' drastic actions. Another flashback shows Derrick's dark side as a bully, shocking the Prefects.
