Takakura Ken (nicknamed 'Okarun' by Ayase) is the male protagonist of Dandadan and the one who was cursed by Turbo Granny in the series' first episode. He is an introvert with an interest in aliens.
His interests led to him getting bullied by some of his classmates, which was the reason why he met Ayase, the female protagonist. As these two started experiencing occult, their days seemed to change. But that was only true for Ayase because Okarun was still getting the same treatment as before.
The strangeness surrounding Okarun is also evident from the fact that no family members or relatives associated with him have been unveiled in the series yet. All of this could hint at him being a supernatural existence, more specifically, a Tulpa. Tulpa is a mystical being from Buddhist mythology said to be created through the spiritual powers of a medium.
Ayase has been obsessed with the Japanese actor Takakura Ken since the start of the series. However, she has never found someone, and her previous boyfriend, who only looked similar to Ayase's crush, had no personality traits similar to Takakura Ken. So, the female protagonist might have created a Tulpa (of the same name as her crush) because she is a spiritualist.
This could also explain why no one cares about Okarun's existence except Ayase or anyone associated with her. It also potentially hints at why no relative of the male protagonist has been introduced in Dandadan.
While Okarun being a Tulpa does sit well with his characteristics, this could also hint at an ending that most fans would not be satisfied with.
