As fans might remember, Ryomen Sukuna was using Megumi Fushiguro's body during the Shinjuku Showdown. Thus, no matter what damage the Jujutsu sorcerers dealt to the King of Curses, all of them were inadvertently hitting Megumi.

Out of all the techniques that were used on Sukuna, the strongest was definitely Satoru Gojo's Unlimited Void. During Gojo's fight with Sukuna, the late strongest sorcerer tried to hit Sukuna with his Unlimited Void on multiple occasions.

Unfortunately, during the first few attempts, Sukuna matched Gojo's domain expansion activation speed and countered them. However, in his fourth attempt, the Six Eyes user successfully activated his domain 0.1 seconds before the King of Curses could activate the Malevolent Shrine. Hence, Gojo finally succeeded in hitting Ryomen Sukuna with Unlimited Void.

Surprisingly, as revealed by Sukuna, he had been using Megumi Fushiguro's soul to take the brunt of the Unlimited Voids to help Mahoraga's process of adapting to the domain expansion. Thus, in all honesty, it wasn't Sukuna but Megumi who was being repeatedly hit by the Domain Expansions. This seemingly caused irreparable damage to Megumi Fushiguro.

As revealed by the manga, Gojo had successfully hit Sukuna with his domain for less than 10 seconds. While it would seem like a short time span, it dealt enough damage to the King of Curses to stop him from using his own domain Malevolent Shrine.

Unfortunately, considering that Megumi's soul was the one that was being repeatedly hit by the domain, he was seemingly damaged irreparably by Gojo. The same was revealed by the alleged Jujutsu Kaisen chapter 269 spoilers.

In the chapter, after Megumi Fushiguro woke up from his slumber and reunited with Maki and his other upperclassmen, he was asked about his condition. Surprisingly, as fans had presumed, Megumi wasn't completely fine. He was still under some effects of Unlimited Void as his head was a bit fuzzy.

This was bound to happen as Unlimited Void flooded its target with boundless raw information, overwhelming them to the point where they were completely immobilized. Considering that Megumi's soul had to go through this for nearly 10 seconds, it made sense why he was yet to recover. It is to be seen if Megumi will recover on his own or if he will require some help from Shoko Ieiri.
