The Plus-Sized Elf anime is an adaptation of a Japanese manga series of the same name, created by Synecdoche. The first episode introduced viewers to the three major characters: Naoe, Elfuda, and Ino. Elfuda needed help to get back into form after being corrupted by the human world. The help that she received, however, was useless in the end.

In the previous episode, viewers were introduced to Tomoatsu Naoe, an employee at the Smiley BOAR Massage Relaxation Center, working under his boss, Akiho Ino.
Ino suggested to go for a drink, but Naoe declined politely as he knew Ino’s habit of drinking in excess and becoming too friendly. At this point, a client arrived for the Diet and Bodywork Therapy program. She was dressed in a tracksuit, mask, sunglasses, and a beanie, and introduced herself as Elfuda. Creeped out by her appearance, Ino assigned Naoe to look after the client.
Naoe was intrigued by Elfuda’s green eyes, large ears, and unusual name, leading him to suspect she might be an elf. On the other hand, Elfuda expressed concern about her weight gain, mentioning that she could not return to the Kafar forests in her current condition.
Elfuda's beanie slipped off during the therapy session. This revealed her pointed ears, confirming that she was an elf. She then explained to Naoe that she had arrived in this world through a portal from another dimension. In her world, food was simple, and even salt and pepper were considered luxuries.
But the food in this world was so delicious that it caused her to eat in excess and put on weight. In order to go back to her world, she needed to lose some pounds to reach the original shape she arrived in.
With Naoe's help, Elfuda started her weight loss journey. Viewers saw her jogging and shadowboxing. After her workout, she quickly grabbed a pack of French fries, but Naoe stopped her, reminding her of the need to maintain a calorie deficit. To help increase her metabolism, Ino took her to a hot bath. Afterward, Elfuda was thirsty and reached for a soda, but Naoe was watchful and stopped her.
After a month, Elfuda returned to her original shape. She thanked Naoe for his assistance and went back home. Later, when Naoe visited a fast food restaurant, he was surprised to find her enjoying french fries. Her successful weight loss journey had instilled in her the confidence that she could shed weight whenever necessary.
What to expect from Plus-Sized Elf episode 2? (speculative)
Plus-Sized Elf episode 2 will be titled The Dark Elf and the Beautiful Flower. As indicated by the preview images of the next episode, the episode will shift focus from Erufuda to a new elf that invaded Earth, Kuroeda. Just like Erufuda, Kuoeda is also plus-sized but as compared to the former, the latter is worried about her body figure and wants to get slim.
Naoe, the young therapist who treated Erufuda, will have a chance encounter with Kuroeda when they visit a convenience store. Kuroeda will also join Naoe's therapy program and as compared to Erufuda, Kuroeda will be much more conscious about how nothing fits her waist.
