Uzumaki episode 1 was released on the internet on September 29, 2024. The episode partially adapted the first three chapters of the series, while skipping some key details. The episode delved into the story of Kirei and Shuichi as the former noticed the eerie happenings around her, in her home village Kurouzu.

The episode also introduced the parents of both the protagonists as Shuichi's parents were completely consumed by the curse with his father killing himself to achieve the spiral and his mother becoming mentally unstable. Moreover, Azami is also consumed by the spiral at the end after she helplessly tries to get Shuichi under her spell.

Uzumaki episode 1 commenced with Kirie traveling to her village's train station to receive her boyfriend Shuichi who was returning from high school. As they walked through the streets of the village, Shuichi asked his girlfriend whether she discovered some new strange happenings in the village, more specifically centered around spirals.

Shuichi shared the condition of his father, Toshio Saitou, about how the spiral curse consumed him. Kirei found everything Shuichi said hard to believe to which her boyfriend stated that she would believe him once she witnessed everything with her own eyes. Uzumaki episode 1 then shifted to Kirei school where Azami asked her about Shuichi as she revealed the crescent scar on her forehead.

When Kirei returned home, her father asked her to deliver a sculpture piece to Shuichi's house which was requested by his father. As she arrived at the house, Toshio rejected the piece and stated that he could make spirals on his own. He then twirled his tongue into a spiral, creeping out Kirei who ran away instantly.

After Kirei ran away from Shuichi's house, Toshio received another package from a cooper that was custom requested by Shuichi's father. The next day, the death of Toshio Saitou was announced and the cause of death, his family members announced, was that he fell from the stairs.

However, after Kirei asked Shuichi about it, he revealed that it was a lie and that he died trying to become a spiral by breaking the bones of his body and transferring himself inside a wooden tub. As his body was cremated, black clouds started rising in the sky and formed his face, which traumatized Shuichi's mother.

Shuichi's mother was then admitted to the hospital where everything that formed a spiral was kept hidden from her because she could hear her husband calling her to the other side inside the spiral.

This included keeping hands (due to the patterns of fingerprints) and hair (due to the pattern of tied hair) hidden from her sight. However, Shuichi revealed to Kire that at night, his mother cut off her own fingerprints using scissors, indicating that her mental state was reaching an end and she could die in the future.

Earlier in Uzumaki episode 1, Kirei went to pick up Shuichi as usual from the train station but she was also accompanied by her friend Azami. After witnessing Kirei's friend, Shuichi didn't feel good and ran away, stating that she was the spiral itself. Later in Uzumaki episode 1, Azami revealed the crescent scar on her forehead, which had now become a spiral.

Azami, confessed by one of her deciding followers, rejected him once again, and upon him stating that he was willing to do anything, she asked him to call out Kirei to the park in the future. After the death of Shuichi's father, Azami's forehead scar became even worse and consumed a big part of her face.

The next day, as Kirei traveled to the park, she crossed paths with Azami on the way and witnessed her forehead scar which had now consumed half of her face. Azami then entered the park where her follower asked for a payback for calling out Kirei. However, Azami denied everything and stated that she doesn't need anyone now.

The follower punched her, which removed her hat, and revealed that her forehead scar had now completely consumed her face. The scar sucked the boy inside it and even consumed Kirei, leaving behind only her impression on the ground. After this, Kirei realized that this was only the start of things.
