The Five Kage in Boruto: Two Blue Vortex manga are Chojuro, Kurotsuchi, Darui, Gaara, and Shikamaru Nara. Out of the Five Kage, the first four have remained the same since the start of the "Naruto Next Generations" manga, while the position of Hokage has observed a temporary change due to the events that took place at the end of the previous manga.

Chojuro is the sixth Mizukage of the Kirigakure, the Village Hidden in the Mist. He previously acted as a retainer to the fifth Mizukage Mei Terumi and is one of the last generations of Kirigakure's Seven Ninja Swordsmen of the Mist. He is a powerful swordsman capable of wielding Hiramekarei, one of the mystical swords of the Seven Swordsmen of the Mist.

Kurotsuchi is the fourth Tsuchikage of the Iwagakure, The Village Hidden in the Stones. She is the great-great-granddaughter of the First Tsuchikage Ishikawa and the granddaughter of the third Tsuchikage Onoki, her predecessor to the position. She took on the mantle of the Tsuchikage instantly after Onoki retired from the position.

Darui is the fifth Raikage of the Kumogakure, The Village Hidden in the Clouds. He was previously a student of the Third Raikage and acted as the right-hand man of the Fourth Raikage, his predecessors to the position.

As for Gaara, he is the fifth Kazekage of the Sunagakure, The Village Hidden in the Sand. He was the youngest son of the Fourth Kazekage Rasa and the Jinchuriki to the One-Tailed Beast Shukaku. While he is no longer a Jinchuriki, he has remnants of Shukaku's chakra within him.

Lastly, Shikamaru Nara is the interim Hokage of the Konohagakure, The Village Hidden in the Leaves. While Boruto: Naruto Next Generations began with Naruto Uzumaki as the Seventh Hokage, its ending saw Naruto and his wife Hinata get trapped within another dimension by Kawaki.

However, with Kawaki using Eida to lie to everyone that Boruto killed Naruto, the village had to choose a new Hokage. Considering that Shikamaru Nara was a capable Shinobi and the chief aide to Naruto when he was the Hokage, Konohagakure picked him as the next Hokage. However, Shikamaru has yet to take the title of the Eighth Hokage officially.
