Simply put, no, you don't need a PS Plus subscription to play the Black Ops 6 Campaign on the PS4 or the PS5. The only requirement is that you must have an active internet connection to be able to play the Campaign mode. So, if you intend to play only the single-player narrative experience, you won't have to spend a buck to enjoy this mode as long as you have an internet connection.
Call of Duty announced that this year's campaign mode will need an active internet connection to run as the game will be constantly downloading texture files to boost the visuals. This was done in an effort to save the total install space on the system, which can go over 100 GBs. Hence, you must have an active internet connection if you want to play the Campaign, although you won't need a PS Plus subscription to get started.

Which Black Ops 6 modes need a PS Plus subscription?
While the Black Ops 6 Campaign doesn't need a PS Plus subscription, you'll need it to play Multiplayer and Zombies. The basic tier (Essential) of PS Plus will get the job done if you want to play online and you don't need to spend any money on higher-tier subscriptions such as the Deluxe or Extra.
If you are wondering what happens to Warzone after Black Ops 6 integration, then you don't have to worry as Warzone will remain a free-to-play game and thus won't require any PlayStation Plus subscriptions.

That said, Call of Duty has yet to clarify whether Split Screen multiplayer in private lobbies will need an active subscription for either of the users. Moreover, currently, it is not known whether there will be an offline Zombies mode, which will allow you to play the round-based modes solo.
Be that as it may, considering that even the Campaign needs an internet connection, it is highly unlikely that there will be an offline Zombies mode.
