This is one of the new Talismans that you can find during your travels around the Land of Shadow. With this Talisman equipped, you will get an increase in damage if you attack after holding the same stance for a while. The time duration to activate the buff from this Talisman is indicated by a red aura that surrounds the Tarnished.

You can find Rellana's Cameo in Castle Ensis. The Talisman is located right behind the NPC invader, Moonrithyll, who you'll have to defeat before you can get your hands on the Talisman. Moonrithyll is fairly easy to take down; any decent bleed build should be able to get the deed done in a matter of seconds.
While you're in Castle Ensis, you can also get your hands on the Milady Light Greatsword in Elden Ring Shadow of the Erdtree. Additionally, you can find the Wing Stance Ash of War, which is located just past the Castle-Lord's Chambers Site of Grace. This Ash of War gets a hefty damage boost when used with Rellana's Cameo.
You can also pair this Talisman with the Shard of Alexander to get a 32% increase in damage. Now, you can use this combination of Talismans with the Great Katana, Moonveil, Sword of Knight and Flame, and Rellana's Twinblades in Elden Ring Shadow of the Erdtree. These are already some of the hardest-hitting weapons in the game, so combining them with Rellana's Cameo and the Shard of Alexander can produce amazing results.
It is easy to get into the habit of holding stances while using Rellana's Cameo. However, this is something that should only be done when you're certain you have enough time to hold your stance and get your attacks off safely.
