To begin this Side Adventure in the game, head to the northeastern part of Death Mountain and into Deep Akkala. Head as far east as possible, and you will see a road. Get on the road and travel north. Soon, you will run into a crossing where upon taking a right, you will encounter a young girl named Loone. Talking to her will set this side quest in motion.
Another simpler way of reaching the same place is by fast-traveling to the Jochi-iu Shrine and taking the road downhill. However, this requires you to have already visited and completed that Shrine, making the first method more accessible in The Legend of Zelda Tears of the Kingdom.
Loone will ask you to head to the northern part of Hyrule in The Legend of Zelda Tears of the Kingdom and replace a fossil's lost eyeball into its eye socket. You can get to this place by heading north from Eldin Mountain to the end of the game's world. Alternatively, you can head east from Mayak Shrine after fast-traveling if you have the option to do so.
Upon reaching the quest's location, you will see the fossil with the missing eyeball. The missing body part lies underneath the skeleton and is stuck inside several rocks. Break these rocks using a sword fused with a rock or shoot a Bomb Flower with an arrow at it.
Once the eyeball is freed, you have to use your Ultrahand ability to move it from the place and replace it in the fossil's socket. This might require some effort but it will eventually go in and stay there.
Doing this will please Loone, whom you will find standing next to the fossil all this while. She will give you a Purple Rupee for completing the quest, which you can use to purchase items in The Legend of Zelda Tears of the Kingdom.
