The Spitter SMG cannot be looted from regular enemies. You'll need to start a quest named 'A Big Score' for an opportunity to receive it. This side quest can be started after you gain access to the Slag Heap settlement within the Garbage region.
Talk with a man named Zakhar Babay inside Slag Heap to begin the Stalker 2 quest. He'll ask you to retrieve his money from a debtor. Zakhar needs your manpower so that he cracks faster. Upon reaching the location, you will be presented with a scene of Zakhar's thugs beating the "debtor." The cronies will ambush you as soon as you get close to them. Kill the bandits and the debtor will call for your help.
