To obtain what is best described as "the horrible ending," you must complete the Three Fingers sidequest. To accomplish so, return down the same sewers that the Dung Eater's quest had you sludging through. It's a dark and deadly journey that takes you to the Cathedral of the Forsaken via a dungeon.
You'll fight a boss and then platform down a jumping challenge to the bottom of a shaft, where you'll find a Site of Grace. Remove all of your armor and weapons and the Three Fingers will hug you. Fight the final boss while the Three Fingers are still on you and... see what happens.
How to get the "good/best" ending in Elden Ring?
To get the best ending possible – best meaning the best conclusion for your character, not necessarily the most pleasurable to watch – you must complete Ranni's quest line completely. As previously stated, this side quest begins at Ranni's Rise in Liurnia of the Lakes and it's a big one.
The side-quest introduces gamers to a few characters while taking them through two fallen cities, as well as a literal country of rot. This questline also has a number of bosses.
