Elden Ring Shadow of the Erdtree, much like the base game, features several weapons, armor, and other combat-oriented trinkets, including new Spirit Ashes, for you to collect as you explore the Shadow Realm. The Bloodfiend Hexer's Ashes, while not as impressive as a new weapon, is still something you won't want to miss out on, especially for the associated boss fight.
You get the Bloodfiend Hexer's Ashes after completing the Rivermouth Cave, one of the few optional dungeons scattered across the Land of Shadow. The Bloodfiend's Spirit Ash isn't anything noteworthy, especially compared to the Mimic Tear, Tiche, or even the other more powerful Spirit Ashes you can get in the expansion.

Where to find the Bloodfiend Hexer's Ashes in Elden Ring Shadow of the Erdtree?
The Bloodfiend Hexer's Ashes is essentially a reward for defeating the Chief Bloodfiend at the Rivermouth Cave. However, to reach the Rivermouth Cave, you first need to make your way to Scadu Altus. Depending on the boss you choose to defeat first in Elden Ring Shadow of the Erdtree—Divine Beast Dancing Lion or Rellana, Twin Moon Knight—accessibility to this place can be early or quite late into the game's main story.
While you cannot go wrong with either of these as your first Remembrance boss in the DLC, to make your way to Scadu Altus early, defeat Rellana first. Since Shadow of the Erdtree, much like the base game, doesn't force you to defeat bosses in any specific order, it's not a big deal to deal with Rellana first and Dancing Lion second.
Once you reach Scadue Altus, head west to the Moorth Ruins Site of Grace. From the Site of Grac, head northwest to make your way to the Temple Town Ruins.
The Temple Town Ruins is where you will find the Rivermouth Cave; however, you will need to descend right below the ruins to find the entrance to the cave. Thankfully, near the southern plateau of the Temple Town Ruins, you will find a stack of tombstones embedded to the plateau's edge, which you can use to descend to the cave.
Once you reach the Rivermouth Cave, descend into the blood-soaked pool to eventually reach the Chief Bloodfiend. The pool is guarded by a big group of Bloodfiends, dealing with it first before going to the boss is highly recommended. Once you defeat the Chief Bloodfiend, you will be rewarded with the Bloodfiend Hexer's Ashes.
