The game is set in a fictionalized city called Lost Heaven, a rendition of 1930s Chicago with elements of an era-appropriate New York. The gritty world of the Mafia has Vito climb up the ranks, with a thrilling conclusion to his story at the end of the game. The game is available on PS4, PC, and the Xbox Series X | S platforms.

Set in the time just after World War II ended, the campaign takes place across the late 1940s and early 1950s. Vito, a war hero, is dragged into the world of organized crime along with his friend Joe.

The game also has two DLCs, which are included as standard with the remaster of the game. The second Mafia game is set in Empire Bay, which takes inspiration from many major American metro cities of the early 1950s. Vito and Joe are also considered as the most iconic characters in the Mafia games.
