The Cathedral of Manus Metyr is one of the optional areas that you can completely miss in Elden Ring Shadow of the Erdtree. Although the route to the Cathedral is accessible right after you reach the Highroad Cross, it's hidden behind Bonny Village, which itself is another optional area of the game, and isn't directly visible on the map of the Shadow Realm.
While optional areas in Elden Ring Shadow of the Erdtree aren't necessary to progress the main storyline, they do feature some of the most memorable boss fights in the game. The Cathedral of Manus Metyr contains the Metyr, Mother of Fingers boss fight, as well as the questline involving the Fingers and Count Ymir.

Steps to reach Cathedral of Manus Matyr in Elden Ring Shadow of the Erdtree
Your journey to reach the Cathedral of Manus Metyr starts as soon as you arrive at Scadu Altus and the Highroad Cross Site of Grace. However, to reach this Site of Grace, you first need to defeat one of the major bosses of Elden Ring Shadow of the Erdtree—Rellana, Twin Moon Knight. Much like any other boss in the DLC, Rellana is quite challenging.
Once you defeat Rellana and make your way to the Highroad Cross, head to the East to reach Moorth Ruins Site of Grace, which is going to be the checkpoint from where you make your descent to the Bonny Village. From the Moorth Ruins, head north until you reach a cavern-like structure among the rubble.
Head down carefully, preferably while using your Spectral Steed Torrent, and you will eventually reach a ladder. Scale the ladder to arrive at the Bonny Village Site of Grace. From there, head straight north until you reach a cavern-like area guarded by a Furnace Golem. You can choose to fight the Furnace Golem here or skip past it.
If you do choose to fight the Furnace Golem, you must use the Spirit-spring near it to ascend on a plateau and throw a Hefty Furnace Pot into the Golem's head. You will need to throw two Hefty Furnace Pots to defeat the Furnace Golem. Alternatively, you can choose to attack its head using ranged spells to kill the Golem.
Once the Golem is defeated, head straight north and you will reach the Church District Highroad Site of Grace. From here, you need to take the road to the left, through the forest, which will eventually take you to the Cathedral of Manus Metyr. Head inside the structure to unlock the Cathedral's Site of Grace.
Inside the Cathedral of Manus Metyr, you will meet Count Ymir, a sorcerer who seems to have devoted his life and loyalty to Metyr, Mother of Fingers. Count Ymir's questline is quite lengthy, but it rewards you with what is essentially the most important lore revelation in Elden Ring, something you definitely don't want to miss out on.
