Herbert Dean, considered the gold standard for referees in mixed martial arts, was a fighter as well from 2001 to 2007, in which he had five bouts, winning two and losing the rest in the heavyweight division (206-265 lbs.).

As a referee, he worked in various promotions, including the UFC and One Championship, and has been praised by Dana White, Joe Rogan, ESPN, Fox Sports and many more. He also won the Referee of the Year award at Fighter's Only Magazine's World MMA Awards from 2010 to 2023 in a row, with the exception of 2015.

Dean has the impression of being right most of the time with his decisions and stoppages, but for the same reason, he has always been surrounded by controversies. In 2004, he refereed UFC 48's Tim Sylvia vs. Frank Mir bout, which he stopped in the first 50 seconds due to an armbar causing Sylvia's forearm break. Sylvia strongly protested against it during and after the fight; however, ringside physician and X-ray reports clearly show his forearm to be broken. Later on, Sylvia acknowledged that he knew his arm had broken but wasn't willing to leave the fight.

Similar to this, he had several controversial fights; however, the most recent and most anticipated fight he refereed was UFC 229 in 2018, Khabib Nurmagomedov vs. Conor McGregor, in which he jokingly conversed with Khabib about how much was he paid for not calling out McGregor's illegal moves.

A few of the other controversies include UFC 169, Urijah Faber vs. Renan Barao, UFC Fight Night: Hunt vs. Oleinik, C. B. Dollaway vs. Khalid Murtazaliev, and UFC 235, when Ben Askren used a bulldog choke to defeat Robbie Lawler, and Dana White criticized Dean for giving him the victory.
