Alexa Grasso joined the UFC in August 2016, winning her debut against Heather Jo Clark in November.

In February 2017, Grasso faced her first career loss against Felice Herrig. Despite facing setbacks, she secured some impressive wins, including a catchweight victory over Randa Markos in August 2017 and a unanimous decision victory against Karolina Kowalkiewicz in June 2019.

In September 2019, she earned the Fight of the Night award but lost by a majority decision to Carla Esparza. In 2020, she moved up to flyweight, winning unanimous decisions against Ji Yeon Kim and Maycee Barber. In her next fight, Grasso submitted Joanne Wood in the very first round.

Despite fight cancellations and visa issues in 2022, Grasso returned with a win over Viviane Araújo in October. Her historic victory over Valentina Shevchenko in March 2023 at UFC 285 made her the first Mexican UFC women's champion, earning her the Performance of the Night bonus.

Grasso's first title defense took place on September 16, 2023, in a rematch against Valentina Shevchenko at UFC Fight Night 227. Grasso retained the belt via a split draw by the judges' scorecard.
