Sam McCall was born in 1980 to Alexis Davis but was immediately given up for adoption at her birth. She was adopted by Cody McCall, a con artist featured on General Hospital. Sam ended up learning Cody's tricks and becoming a con master herself.

The character first appeared in Port Charles in 2003, working with Jasper Jacks. She later had an affair with Sonny Corinthos and became pregnant with his child. However, Jason Morgan claimed the child as his own to save Sonny's marriage, and Sam prepared to give birth to their daughter, Lila.

However, Sam lost her daughter after Alexis pleaded with her to donate stem cells to Sonny's ailing daughter, Kristina. Despite this, Sam and Jason fell in love and got engaged.

Later, Alexis and Sam found out about the adoption and that Alexis was Sam's biological mother. After a heated argument with her mother, Sam got drunk and slept with Alexis' husband, Ric Lansing. Sam and Jason also split apart after her out-of-control behavior when she found out Jason was the father of Elizabeth's son Jake.

Years later, Jason briefly went to prison to cover up for Michael, and Sam found herself missing him. The two got back together after his release and ended up getting married. She even got pregnant but gave birth to a stillborn child once again. Later, it was revealed that Heather had kidnapped and switched the baby. The couple reclaimed their son and named him Daniel Edward Morgan. Later, Sam also found out the identity of her own biological father, Julian Jerome.

Eventually, Sam and Jason broke apart due to a brewing mob war and her worries about the safety of her children. Despite the split, the couple continued to co-parent their family together. Sam finally got over Jason when he married Carly and she started getting close to Dante, bringing an end to one of the supercouple relationships on General Hospital.
