Audiences are anticipating Smile 3 to be even bigger than the previous films in the Smile film franchise, specifically after how Smile 2 ended. Those who have watched Smile 2 will already be aware of Skye Riley's (the film's main protagonist) fate towards the end, and to put it crisply, it wasn't good.
Just like the first film in the franchise concluded with the protagonist's death, Smile 2 also ended with the title character's demise. However, what sets their deaths apart is the number of people who witnessed it.
While Dr. Rose Cotter's death in the first installment of the film (released on September 30, 2022) was witnessed solely by Joel, Skye Riley's death in Smile 2 was witnessed by thousands of people present at her concert. Since the entity has a habit of moving from one person to another following the observation of the prior's death, each one of the audience members is at risk of being affected by it.
It is also possible that the entity picks and chooses the most vulnerable among the lot and latches onto them as it tends to attach itself to people who have a traumatic background.

Smile 3 is expected to be on a much larger scale than Smile 2
Despite Smile 2's definitive ending, many people are confused about it. This is because several scenes in the film were revealed to have not actually happened by the movie's end.
Some of the audience members believe that just like Skye's mother's death was revealed as not real but the entity's way of messing with Skye's head, it is possible that Skye's death is not real too. In that case, the audience hopes she is alive in Smile 3.
However, if Skye did die in Smile 2, the pool of victims the entity can choose from in Smile 3 is large. When Skye walked up on stage and shoved a microphone to her face, killing her in the process, the act was witnessed by thousands of people who were part of the audience at her concert.
The act was also encountered by many other people watching from their homes and other locations through screens. This implies that each of them could be the next victim or victims if Smile 3 plans to get larger. This is likely to happen, as highlighted by writer and director Parker Finn during a conversation with The Hollywood Reporter on October 19, 2024.
Talking about the scope of the potential Smile 3 movie, he said:
"I love this idea that potential future iterations of Smile could go bigger and more off the rails and even crazier and more unhinged."

The film is currently only available in theatres. The film is distributed by Paramount Pictures and hence the audiences expect it to be made available on Paramount+ sometime soon.
The audience has been showing great enthusiasm for Smile 2 and is eagerly anticipating an announcement regarding Smile 3, which is yet to be confirmed.
