The cast features Paul Mescal as Lucius Verus Aurelius, popularly known as "Hanno," a man seeking vengeance after his wife is slain by Roman soldiers. Denzel Washington plays Macrinus, a former gladiator who seeks to dominate Rome.

Connie Nielsen plays Lucilla, Pedro Pascal is General Acacius, and Joseph Quinn and Fred Hechinger play Geta and Caracalla, the co-emperors. Sir Derek Jacobi reprises his role as Senator Gracchus.
