What happened in Earth Abides episode 5?

Though all of them willingly agree to do so, Silas rejects the decision and leaves the gates, which he immediately regrets. Ish killing Charlie and expelling Silas out of the community, for the first time, establishes the rule that any perpetrator committing a horrendous crime will receive death.
Earth Abides episode 5, after a brief time leap, brings another grave threat to the scenario when Ish's son Joey suddenly runs a temperature and suddenly falls very ill. He collapses on the floor and is immediately admitted to the medical room. Slowly, more people from the community begin to fall sick that very night and Molly reveals to Ish that the deadly virus, which had wiped most of the population from Earth, has returned back with a different strain.
This time, it is even affecting people who had survived in the initial phase, which can pose a grave danger to the remaining community members. Ish had developed a very close-knit relationship with his youngest son, Joey. He considers Joey to be the heir who could lead the civilization into the new world. Joey getting affected by the disease poses a threat to the entire next phase of humanity.
This breaks Ish down deep inside, and he embarks on a journey to retrieve the remaining medical supplies from the medical ward across the bridge. Earth Abides episode 5 is emotion-heavy; it leaves the audience wondering about the possible future of mankind.
