Fast forward to Lois, who is visibly drunk, leaving the hospital in her car. She crashes into a pole, and the next time she wakes up, she is in the hospital and handcuffed to the bed. Her "nurse" arrives, and it is none other than Eddie, whom she pleads to help her escape. Eddie and Lois are seen running around the hospital hallways as they try to escape the hospital and into a red convertible.
In episode 4, Detective Lois gets a new clue about Mr. Grotesquerie, which brings her and Sister Megan to a remote location that resembles the fire pits of hell.

Grotesquerie season 1 episode 4: A new clue brings Detective Lois and Sister Megan to fire pit where they meet Dr. Joseph Ritter
A clue from one of the earlier murders in the Grotesquerie Season 1 premiere remains unsolved in episode 3. However, the fourth episode uncovers the secret of the wooden box that Detective Lois Tryon brings back home. Her initial intent to bring it home is for her daughter, Merritt, to help her solve the puzzle.
In Grotesquerie season 1 episode 4, Merritt finally opens the wooden box, and when the detective asks her what is inside, she gives him a tiny piece of rolled paper. Detective Lois opens it to find a series of coordinates of someplace. She then calls Sister Megan, telling the nun slash reporter that she has another lead about the serial killer.
They drive together in the detective's car to go to the place the coordinates are pointing. On their way, Sister Megan reveals that she was once a promising opera singer who could have entered Julliard with her rendition of I Don't Know How to Love Him from the original Broadway production Jesus Christ Superstar. The detective later joins her, and the two spend the entire ride having a mini karaoke session.
However, their fun musical ends when they arrive at the location as Detective Lois continues to hunt the killer—a fire pit in the middle of nowhere. The fiery pit is a literal and metaphorical gateway to hell, which gives the detective and nun duo more questions than answers.
At the location, they meet Dr. Joseph Ritter, who explains that the area has been hit by earthquakes, and with the natural gas deposits, the result is the fiery pit that's only getting bigger by the day. Ritter also implies that the picture is like "hell opening up," which only adds to the sense of dread between Detective Lois and Sister Megan.
As the duo attempts to look for refuge after the highway is blocked by burning cars and fire trucks, they meet a man at a gas station who warns them about the effects of climate change. However, before that, an imagery of the Last Supper (as depicted in the murder in episode 2) appears when Lois leaves a bill on the countertop to pay for gas.
What did Sister Megan confess to Detective Lois?
Ryan Murphy's Grotesquerie season 1 episode 4 reaches its climax as Detective Lois and Sister Megan drive home from the gas station. There's almost no visibility in the road, and the nun is rattled. When the detective tells her that she won't allow anything to happen to her, Sister Megan confesses that she has sinned, referring to Father Charlie.
Earlier in Grotesquerie season 1 episode 4, Sister Megan visits Father Charlie in the church, where she sees the father spin cycling topless. Their conversation about sinning lead to the nun and the priest having s*xual relations after they deduce that, as humans, they are and will always will be sinners.
Back to the detective and nun's conversation, when Lois mentions that fancying Father Charlie is not a sin, Sister Megan further admits that it wasn't merely a look that she did. She tells the detective that she also acted on it, calling it the greatest sin she could commit after promising celibacy when she became a nun.
During Sister Megan's confession to the detective, she thinks that everything is her fault because she has sinned. She believes that she is one of the responsible why the town suffering with horrific murders. Her confession is a poignant part of Grotesquerie season 1 episode 4, until a thrilling and terrifying conclusion in the episode.
In the middle of their heartfelt chat, Sister Megan suddenly shouts for Detective Lois to stop the car. The detective slams the brakes before she hits a blood-soaked woman. When they tell her to hurry and get inside the car, the bloodied woman creepily tells them that they need to leave quickly because "He" is coming.
