In Outer Banks season 4 part 2, Madelyn Cline's character Sarah Cameron learns she is pregnant. She realizes that she must tell her husband John B about the baby, but feels apprehensive about handling the situation, thinking that the couple might not be ready to raise a child yet.

This reveal comes after many fans speculated that Sarah Cameron might be pregnant at the premiere of the series trailer, during the Poguelandia fan event in Santa Monica.

How was Sarah's pregnancy revealed on Outer Banks season 4?

Outer Banks season 4 confirmed Sarah's pregnancy in episode 7 of the season after she took two pregnancy tests in a bathroom. Both of them came back with positive results. When Sarah found out, the first person she confessed to was Kiara, who came to the bathroom to check in on her.

Sarah further confessed to Kiara that she was extremely nervous about raising a baby. She recalled previous events in the season, such as the fight to retain their own land and their ongoing hunt for Blackbeard's treasure. Kiara agreed with her concerns, however, she assuaged Sarah by telling her she would make an amazing mother.

Kiara also encouraged her friend to give the news to John B, since he loved and supported her. Furthermore, Sarah realized that she would have the support of all her friends in raising her child.

In episode 8 of the series, Sarah decided to tell her husband about her pregnancy. Even though John B was nervous as well, with the same apprehensiveness as Sarah, they decided to stick by each other and figure it out, as they do with everything else.
