Outlander season 7 episode 12 was a big challenge for the show's hero, Jamie Fraser, after he learned about what happened when he was gone and presumed dead. The episode is titled Carnal Knowledge, referencing the emotionally gutting scene of Claire and Lord John Grey having s*x over their shared grief for Jamie's death in Outlander season 7 episode 11.
In the latest episode, Jamie is faced with some decisions to make that will alter the course of his marriage to Claire and his friendship with Lord John Grey. Learning that his wife had s*x with his friend, no matter the reason, left Jamie seething in jealousy. The revelation led him to lash out and act out, from punching John in the face to calling Claire a loose woman, but while his friendship with John hung in the balance, Jamie decided to forgive Claire and move on from what happened.

Outlander season 7 episode 12 saw Jamie decide to surrender Lord John Grey to the rebels
Jamie Fraser first learned that something carnal had happened between his wife Claire and Lord John Grey when the latter confessed about it while they were in the woods. Jamie had escaped the Red Coats while pretending to kidnap John, and while in the woods, John told him:
"I have had carnal knowledge of your wife."
Jamie was calm, if not a bit confused, because he knew that Jamie had a preference for men. He also scoffed at the idea that Claire would be a willing participant in what happened, knowing how much Claire loved him, which incensed John. However, John lets him have it, saying that he and Claire were both "f*cking" Jamie that night, and that's when Jamie punched Lord John.
The assault continued until some continental soldiers/rebels interrupted them. Jamie told them that John was a prisoner, but they were suspicious, especially when one of the soldiers said that John was a uniformed officer. They announced that they would take John, which had Jamie almost pulling a muscle. He knew that handing John to the rebels, who were likely going to torture him if not hang him, was a life-altering decision to make.
However, when the rebels shifted their suspicions toward Jamie, he quickly handed John over. This decision was monumental in terms of Lord John's future because his fate hung in the balance once he was brought to the rebel camp.

Jamie decides to work things out with Claire in Outlander season 7 episode 12
Learning what happened between Lord John and Claire was a major plotline in Outlander season 7 episode 12. It served as one of the major hiccups in Jamie and Claire's relationship in the seventh season of the series, and he's faced with a major decision—to forgive Claire and move on from what happened or allow that night to put a strain on their marriage.
In the end, Jamie decided that he couldn't live without Claire in his life, and so they reconciled. However, it wasn't easy for the two to decide to fight for their relationship. Their confrontation made for some of the tensest moments in Outlander season 7 episode 12. At one point, Jamie also inquired about the gory details of that night, albeit callously. He asked her:
"Did he bugger you?"
What does bugger mean? Per the Cambridge dictionary, bugger is a UK offensive word used to call someone who is silly, annoying, or difficult. It's also used as a slang or a ride phrase to express great anger, like "Oh, bugger!" or great surprise, like "bugger me!" However, the word takes on a different meaning in the series, so what does bugger mean in Outlander? It means committing sodomy with another person.
It looked like an*l s*x was a step too far for Claire because when Jamie inquired if her night with Lord John Grey included some "buggery," Claire was taken aback. She called Jamie an "absolute b*stard" before walking out.
