As per Merriam-Webster's online dictionary, fetid is defined as something 'having a heavy offensive smell,' while the word moppet is an endearing term for a 'young child.' So, Jame Eagan's strange put-down literally translates to a 'smelly child.' But it's much more plausible that he meant to call her something along the lines of a spoiled brat.

Unpacking the meaning of Jame's insult from Severance season 2
The unique insult has caught the attention of Severance fans, who are quite fascinated by the odd term and its meaning. 'Fetid moppet' is not a part of modern vernacular, but these words were used centuries ago. Fetid, derived from the Latin word fetidus or foetidus, dates back to the 15th century, while moppet dates back to the late 17th century.
The hit Apple TV series is known for its unique turns of phrase; after all, Milchick was called a 'shambolic rube' in the previous episode. Also, the Eagan family's communication style strongly hints at their cult-like presence. Another thing to notice is how Helena recoils away from her father after being called a 'fetid moppet,' indicating that she has been used to his angry outbursts from a young age.
Moreover, it's not clear if Jame Eagan's choice of words was aimed at Helena or Helly. His disgust for her innie was well established in season 1, episode 9, titled The We We Are. In the episode, the father-daughter duo shared a sweet moment after Jame learned of Helly's suicide attempt on the severed floor. While checking up on his daughter, he berated the innie for her callous actions by saying:

What happened in Severance season 2 episode 2?
Severance season 2 episode 2 deals with the immediate aftermath of the season 1 finale. The first season ended with Helena's innie making an announcement to guests at the Lumon gala, which was held to promote the severance program. She revealed that the innies were miserable and tortured on the severed floor.
Before she could make any more shocking revelations, the overtime contingency was ended (by Milchick from the Lumon office), allowing Helena to take over again. The latest episode picks up right after this incident, showing Jame's angry reaction. It is in response to Helena putting Lumon's reputation at risk, even though technically it was not her fault.
Nevertheless, Helena quickly regains her composure and takes charge of the situation. She orders Milchick to find out the extent of the damage done by the innies during the overtime contingency. Helena and Milchick lure a reluctant Mark to return to Lumon, as his work on the Cold Harbor file is very important to the company.
In a bid to placate him, they also bring back Irving and Dylan, who were previously fired for their innies' role in breaching the severance protocol. Helena also returns to the severed floor, posing as her innie Helly R., to spy on the macrodata refiners and keep them in check.
