Hardcore Holly accidentally became Champion at WrestleMania
If you're going to make a mistake, you may as well do it at the biggest event of the year and get the worst out of the way fast, which is what Hardcore Holly seemingly did back at WrestleMania 2000. The second match of the night saw a number of mid-card talents fight it out for the Hardcore Championship in a 15-minute battle royal, the title was contested under the 24/7 rule which even took place throughout this match so the title changed hands a shocking 10 times throughout.

Crash Holly was reportedly scheduled to retain the title but Hardcore Holly had a better idea and pinned his cousin after hitting him with a jar of sweets. Crash didn't kick out of the pinfall attempt and so Hardcore Holly left WrestleMania as Champion, luckily the 24/7 rule meant that this didn't really matter in the grand scheme of things.
