Possibly the most iconic WWE moment not to have been planned, Mick Foley's fall through the Hell in a Cell structure could have had some dire consequences. King of the Ring 1998 was a rough night for the WWE legend, who was wrestling The Undertaker under his Mankind gimmick inside (or above) the cell.
The Deadman threw him off the top of Hell in a Cell through the announce table, making for one of the most iconic images in WWE history.

What followed was almost as memorable, but it was unscripted. The Undertaker chokeslammed Mankind on the top of the structure and he was supposed to bounce off the cage.
The WWE Legend went through the ceiling and fell into the ring. This was a terrifying moment, one that had gone horribly wrong. However, things could have been much worse. The Undertaker explained how he stepped off the panel that gave way moments before he did. If he hadn't done that, he would have fallen into the ring along with Mankind.
Fortunately, WWE avoided tragedy and Mick Foley did not suffer any serious injuries. He was able to finish the match, with The Undertaker mercifully pinning him in the ring.
