Alicia Fox returned to WWE to participate in this year's Women's Royal Rumble Match. She entered the rumble at number 21. Shortly thereafter, R-Truth, the 24/7 Champion, followed her to ring. He was being chased, as usual, by a band of WWE Superstars.

After she helped him fend off this crowd, Fox took advantage of Truth's distraction. She rolled him up for the pin to win the title to become the seventh female superstar to hold the 24/7 Championship. Unfortunately for her, she was eliminated from the Royal Rumble a few seconds later by Mandy Rose.

As soon as she was eliminated, Truth quickly rolled Fox up for the three count to recapture the gold. Although it was short-lived, Fox's title reign was the second title run in her career. She won the Divas Championship in 2010.
Fox made history by winning the 24/7 Championship because she became the first-ever female African American to hold the gold. Likewise, she was the first and only African American Divas Champion.
