According to a veteran, the most special belt he has is not actually an official title belt. However, it still holds immense value for him.

Sam Houston, is a notable name in the pro-wrestling business. Having wrestled in both WWE and WCW, along with several other promotions in his lifetime, he has certainly left his mark in the industry. He is currently semi-retired, wrestling only a few matches a year at times.
On this week's episode of Sportskeeda's UnSKripted, Houston was asked about his most prized memorabilia, to which he brought out a cardboard belt that was given to him by Bill Apter.

"I mean this is something special for me. I mean not everybody gets a Bill Apter title. And then when I lost everything in the floods in 2016, floods in South Louisiana, I am sitting up in North Texas on Christmas morning and all I had was my wrestling boots, a pack of cigarettes, this belt... And I was happy. And I kept this with me." [5:10 onwards]
Sam Houston recently came out on top in his fight against cancer. It remains to be seen what is next for him down the line.
