On July 31, 2018, he made his Major League debut after being promoted. He was then traded to the Baltimore Orioles on January 3, 2023.He was soon designated for assignment. On April 13, 2023, the Orioles selected his contract. O'Hearn had his first three-RBI game in two years in his Orioles debut, an 8-7 victory over the Oakland Athletics that afternoon. On September 18, 2023 he helped defeat the Houston Astros 8-7 while playing away from home, setting a single-game career high with five hits.
