Having signed with the Houston Astros in 2011 as an international free agent, he began his career with the DSL Astros. He spent his time switching between teams and leagues. In between this Hernández was added to the Dominican Republic national baseball team's roster following the season at the 2015 WBSC Premier 12. He was called up to the Major Leagues on August 12, 2016. However, the Astros traded him to the Toronto Blue Jays in 2017. His is currently an free agent having last played for the Seattle Mariners.

Throughout the years, Hernández has managed to gather many accolades to his name, his accolades includes winning the Silver Slugger Award in 2020 along with MLB All-Star and All-MLB Second Team honors. He won the Silver Slugger Award once more in 2021.
