Currently, the best and most efficient way of getting Baneful Hearts is to loot them from the Tortured Chest. The Tortured Chest has a fair chance of spawning in the Helltides (a special gathering of high-level enemies that can appear on the map as red marks). Facing a Helltide is no easy task, so approach them only if you have a proper build setup.
With Season 4, the Helltides have also gone through some changes and become much more accessible for players. Previously Helltides only appeared on higher Tier worlds but with the new season, they can only appear on World Tier 1. So if you don’t want a difficult game, you can still get approach Helltides and farm Baneful Hearts.
Once you complete a Helltide region and successfully summon a Tortured Chest, you will still need 100 Aberrant Cinders to open them. These can be collected by killing monsters and completing various events in a Helltide-affected region. So keep an eye open for any event that pops up in Helltides.
Another way you can farm Baneful Hearts is by killing Hellborne bosses. These behemoths only appear in the Helltides once you have reached a threat level 3. They are extremely dangerous and can kill you in an instant. But on the bright side, they have a fairly high chance of dropping a Baneful Heart.
Once you have collected three Baneful Hearts, you can use them for the Accursed Ritual. Performing this ritual will summon the blood maiden. She is the main boss of Diablo 4 Season 4 and she is extremely difficult to kill. So keep her for the end game. Killing her rewards a massive amount of XP and loot.
