To get the Voodoo Doll, first, you need to go to the Junkyard in Broken Delta. The coordinates for the location are (3687, -7114). The recommended level for the area is 19, but you can visit at a level as low as 15. Once there, look for the glowing revolver. Interact with it, and you'll be teleported to a warehouse where the Carnival of Doom quest will start.

Upon teleportation, you'll see a Dollar Store Joker threatening hostages tied to chairs, with you among them. The clown shoots the hostages, turning them into stuffed toys, then turns to you and tells you to play his game. The game is Russian roulette; he'll give you a gun with two bullets, and you can either shoot it yourself or make him shoot it.
No matter what you choose, he will win. To defeat him, scan the area and locate a wire. Tell the clown you need some air, and he'll generously let you go.
