She can be found near the shores standing behind a large cauldron. You must bring back her stolen ingredients and put them in the cauldron.

To get the Haunted Rod, put five ingredients in the cauldron. On doing so, you will receive the rod and the Haunted title. It is also worth noting that you aren't required to put all five ingredients together. So, take your time hunting them and bringing them back individually.
