Uzumaki is a Japanese psychological manga series written and drawn by Junji Ito. The story centers around Kirie Goshima, her boyfriend Shuichi Saito, and the residents of Kurouzu-Cho, a fictional town in Japan. The residents of Kurouzu-Cho are plagued by an unknown entity called the spiral which tends to affect those who reside in the town. The people are either drawn to it or completely repel it.
At the conclusion of the narrative, Kirie Goshima's destiny follows the same pattern as that of the other residents of Kurouzu-Cho. Kirie and Shuichi are spiraled together like every other resident in the City of Spirals underneath Kurouzu-Cho.
What happens at the end of Uzumaki?
Together, Kirie and Shuichi stroll down the spiral staircase which appears to have been built centuries ago. Soon, a spiral victim stops them and wishes to join them, but they can't carry him since they're too exhausted. When the spiral victim gets violent, it grabs Kirie and bites her ankle. Kirie is unable to see Shuichi as he rips it off her and gets shoved down the stairway.
As she tumbles down the stairs, she finds herself in the City of Spirals, the capital of the sentient, evil Spiral that has been destroying their town. She falls softly into the city, composed solely of the bodies of Kurouzu-Cho's spiral-cursed citizens, all facing the enormous spiral that towers over them.
Kirie discovers her parents twisted together and turning to stone. She hears Shuichi calling out to her. He feels they are getting close to breaking the spiral curse, so he urges Kirie to carry on without him and keep battling.
He tells Kirie to go on without him. But Kirie, too, is enveloped in Shuichi's arms because she has lost the will to resist the spirals. They are both stuck in time, their hands locked together, their arms partially spinning. It is implied that they will have the ability to move "when the eternal spiral awakens once more," which will be in a few hundred years.

What is The Spiral Curse?
Every few centuries or millennia, at irregular periods, the whole population of Kurōzu-Cho is mercilessly wiped out by an elusive and obnoxious entity known as The Spiral. It is believed to be able to infect individuals and take control of their bodies, and emotions.
Kurouzu-Cho is the only town impacted by the Spiral, and it eliminates the town in a predictable way: first, there are "random" deaths and disappearances, accompanied by "strange" cases of obsession with the pattern; after that, there are more severe effects such as multiple hurricanes and tornadoes; finally, Kurouzu-Cho is rebuilt as an enormous spiral row-house, and everyone inside is sucked into the City of Spirals.
